 The North West Community Care Access Centre (NWCCAC) is one of fourteen CCACs in Ontario dedicated to supporting and enhancing the quality of life, independence, health and well-being of individuals in the community by offering a single point of access for community services and demonstrating leadership and excellence in community care.
NURSE PRACTITIONERS – PALLIATIVE CARE THREE FULL TIME POSITIONS The Nurse Practitioner – Palliative Care has a key interdisciplinary, collaborative role in the delivery of palliative / end-of-life care. Qualifications: Registration in the Extended Class with the College of Nurses of Ontario Enhanced clinical skills and knowledge of full scope of practice Experience, ideally in community health Very strong collaborative skills and the ability to build effective relationships, both internally and externally Ability to work independently Demonstrates a commitment to personal and client safety Advanced written and oral proficiency Bilingual in French an asset Must have valid driver’s license and access to a vehicle. Travel will be required. The NWCCAC serves a diverse population throughout Northwestern Ontario.
