| Description |
 One of a network of over 75 provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by the Ministry of Health and Long-Term Care and the North West Local Health Integration Network, emphasizing access to primary health care and prevention programs and services. The Centre offers an excellent benefits plan and the Health Care of Ontario Pension Plan (HOOPP). All permanent positions noted below are at the Thunder Bay site.
Manager of Allied Health and Outreach Services – 1 FTE The Manager of Allied Health & Outreach Services position will work as a member of a regional management team and be responsible for providing leadership for the health centres’ allied health care providers at three sites. The Centre is seeking a dynamic manager to work at a vibrant organization and lead staff in accomplishing strategic outcomes. Emphasis will be placed on building effective partnerships with external organizations, working with access to primary care, prevention programs/services, outreach and allied health services. This position will report to the Chief Executive Officer. Qualifications: • Strong administrative and supervision skills. • A broad knowledge in the areas of community health, primary care, health promotion. • Management experience in a health care setting. • Interest and experience in working with collaborative and multidisciplinary structures to address barriers to health care. • Demonstrated ability to lead change processes. • Masters degree in a related field is an asset or a combination of educational preparation and applicable experience. • Excellent organizational, interpersonal and written communication skills. • Competency with a range of software applications and knowledge with computerized work environments. • Willingness to travel periodically.
Community Dietitian – 1 FTE This position involves Registered Dietitian services at the Thunder Bay clinic site and to communities served by our mobile unit. Qualifications: • University degree in foods and nutrition, and completion of Dietitians of Canada accredited Dietetic Internship, preferably with a community focus. • Current registration with the College of Dietitians of Ontario. Membership in Dietitians of Canada desired. • Certified Diabetes Educator (CDE) an asset. • Demonstrated ability to work effectively in multi-disciplinary team environment. • Strong interpersonal, organizational and group facilitation skills. • Proven experience working in community based programs and ability to implement participant driven programs. • Awareness of and sensitivity to socio-economic and cultural issues in nutrition. • A valid driver's license will be required to operate the medical and diabetes mobile units. • Ability to work flexible hours and travel to communities served by the mobile units. • Individuals with additional diabetes education and training an asset. • Ability to work effectively with marginalized and under serviced populations.
Health Worker (Case Management) – 1 FTE We are currently seeking a Health Worker – Case Management, who as a valuable member of the Community Health Centre team, will provide intensive case management for clients with complex and multiple needs in the communities that we serve. Qualifications: • Ability to assess, plan, facilitate, advocate for options and services, and to support clients to meet their needs. • Ability to work both independently and collaboratively with a multidisciplinary team. • A strong understanding of the determinants of health framework. • Strong interpersonal, organizational and group facilitation skills. • Ability to work flexible hours. • A current driver’s license and access to a reliable vehicle. • Ability to work effectively with marginalized and under serviced population. • Bachelor’s degree in Social Work (HBSW) or Registered Practical Nurse diploma (RPN).
Therapist – .5 FTE We are currently seeking a Therapist who will provide individual, couple and family counselling, facilitate groups and promote good mental health via a variety of programs and activities. Qualifications: • Graduate degree in health or a social science related discipline. • Specific training in counselling, with a minimum of 3 years experience. • Excellent interpersonal and communication skills, and ability to work in a multidisciplinary team environment. • Demonstrated ability for problem solving, analysis, evaluation, implementation and follow-up of decision. • Ability to work effectively with diverse clients with multiple concurrent problems and a variety of issues. • Ability to work individually with clients, and in a group setting. • Proficiency in the use of computers and software applications. • Awareness of and sensitivity to socio-economic and cultural issues.
Registered Practical Nurse – 1 FTE As part of the clinical team, the RPN, uses the principles of client centred care within the context of a self-directed and interdisciplinary team approach, providing clinical care, illness prevention and health promotion within a community health centre environment. Qualifications: • RPN with current registration with the College of Nurses of Ontario. • Minimum 3-5 years nursing experience. • Strong organizational, communication and leadership skills. • Ability to work both independently, and collaboratively, within a multi-disciplinary team environment. • Perform clinical activities according to the College of Nurses standards for nursing practice. • Utilizes professional assessment skills to identify physical, mental, and psychosocial needs of clients. • Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction. • Proficiency in electronic charting. • Valid Ontario driver’s license.
