Case Management Worker
1 Full-Time (35 hrs/week)
This position is within OPSEU local 720 Bargaining Unit (Grade 4)
Start date: As soon as possible

The Case Management Worker will provide assistance to individuals who are at
risk of being incarcerated, have a severe mental illness and also may be homeless
or at risk of being homeless.
KEY RESPONSIBILITIES:
1. Ability to conduct a bio-psychosocial intake focussing on strength and
recovery for individuals with mental health problems requiring housing to
assist in determining appropriateness.
2. Work as member of a team in venues such as case reviews in order to
profile presenting issues of referred clients to develop strategies for
intervention or alternative resources and options.
QUALIFICATIONS:
• Degree or Diploma in Social Sciences
• Three to five years direct experience working with individuals with serious
mental illness and addictions issues who are not effectively engaged with
formal services (homeless)
• Demonstrated ability to work with complex populations
• Ability to work in unstructured environments
• Ability to travel and must have use of personal vehicle, Valid Driver’s
license, and two million dollar liability
